It is necessary for the City Council to approve Change Order No. 22 for this project as the amount of this Change Order exceeds the $25,000 authorization limit granted to the City Manager. Change Order No. 22 includes the installation of new landscaping, irrigation system, drainage, decorative pavers and associated appurtenances in the medians on Tassajara Road between the 1-580 overcrossing COPIES TO: g:hniscprojSi-580 Tassajara\project\agst CCO 22 Authorization DeSilva Gates Construction ITEM NO. and Koll Center Drive. This.work was not included in the original scope of work and was later added to the project to enhance the roadway medians, starting from the freeway overcrossing on Tassajara Road northerly to the existing landscaped median north of Koll Center Drive. Completing the median landscaping with the freeway interchange project will facilitate the installation of landscaping work within Caltrans' right-of-way, which would normally require another encroachment permit and approval by Caltrans. Additional work includes the furnishing and installation of drainage, irrigation, plants and concrete headers, importing and placing toPsoil and sand bedding, installing decorative interlocking paving, and performing plant establishment work in the Tassajara Road median. For this work, the Contractor will be paid the lump sum price of $162,776.31. In addition to the lump sum, extra work at force account (time and materials) includes the removal of temporary asphalt concrete, backfill and soil compaction, and installation of interlocking pavers for $43,661.08. The total cost of $210,173.12 for Change Order No. 22 was subsequently reduced by $11,941.88 due to the decrease in concrete island paving and aggregate base in the median. This deduction puts the total cost of Change Order No. 22 at $194,495.51. Staff recommends that the City Council approve Change Order No. 22 in the amount of $194,495.51. City Project No. 01-12, Federal Aid Project No. RSTPL-5432 (011) TO: DeSilva Gates Construction LP -You are hereby directed to make the herein described changes from the plans and specifications or do the following described work not included in the plans and specifications on this contract. NOTE: This change order is not effective until approved by the Public Works Director. Description of work to be done, estimate of quantities, and prices to be paid. Segregate between additional work at contract price, agreed price and force account. Unless otherwise stated, rates for rental of equipment cover only such time as equipment is actually used and no allowance will be made for idle time. Change requested by: Engineer The last percentage shown is the net accumulated increase or decrease from the original quantity in the Engineer's Estimate. In the medians on Tassajara Road between the 1-580 overcrossing and Northside Drive, install irrigation, planting, drainage, decorative pavers and associated appurtenances as shown on pages 3 through 16 of this change order. Eliminate portland cement concrete island paving at this location. Perform plant establishment for 90 calendar davs. Construction staking for this work shall be provided by Owner in accordance with Section 5-1.11 of the Special Provisions. EXTRA WORK AT AGREED LUMP SUM PRICE Furnish and install drainage, irrigation, plants and concrete headers, import and place topsoil and sand bedding, install decorative interlocking pavers and perform plant establishment work in the Tassajara Road median between station T1M 20+47 and TIM 23+40 as shown on sheets 3 through 16 of this change order. Drainage inlets shall be constructed in accordance with the reqmrements of Section 51-1.02 "Minor Structures" of the Standard Specifications and shall conform to the details and notes shown on Standard Plan sheet D73. Installation of irrigation and planting shall be performed in accordance with Section 10-2 "Highway Planting and Irrigation Systems" of the Special Provisions. Imported topsoil shall conform to the requirements of Section 20-2.01 "Topsoil" of the Standard Specifications. The plant establishment period for the Tassajara Road median landscaping shall be TYPe 2 and shall be 90 calendar days. Plant establishment work shall be perfgrmed in accordance with the requirements of Section 10-2.04 "Highway Planting of the Special Provisions. Excavation, compaction and backfill, concrete headers, root barrier~ valves, conduit, 18" geocomposite drain, class I concrete, bar reinforcing steel, miscellaneous iron and steel, shall conform to the Standard Specifications and Special Provisions. Traffic control shall conform to the requirements o£Section 10-1.25 "Traffic Control System for Lane Closure" of the Special Provisions. For this work, the Contractor will be paid the lump sum price of $166;512.04. This amount constitutes full compensation, including overhead, waste, pallets, delivery, taxes and markups for the work of this change Lump Sum Price = $166,512.04 EXTRA WORKATFORCE ACCOUNT Place 75 mm of Class 2 aggregate base in the area below the interlocking pavers as shown on plan sheets HP-4a, HP-4b and HP-1 Ia of this change order. Class 2 aggregate base shall conform to the requirements of section 26-1.02A "Class 2 Aggregate Base" of the Standard Specifications and shall be compacted to 95% relative compaction. Fumish decorative interlocking pavers for the work described above and as shown on sheet HP-11 a of this change, order. The color, size, shape and texture of pavers shall match the existing pavers located in the Tassajara Road median at TIM 23+40+. The Contractor shall submit sample pavers to the Engineer for approval prior to installation. Backfill and compact median planters will native soil to a depth of 18" below the top of curb. The soil shall not contain any asphalt concrete grindings, broken concrete or debris. Backfill 'material must be approved by the Engineer prior to use. Contractor shall control backfilling operations to protect existing curb and newly paved roadway. Remove temporary asphalt concrete (cold mix) and place hot mix asphalt concrete in irrigation cross over trenches b~tween median islands. Asphalt concrete mix shall conform to the requirements of Section 10~1.48 "Asphalt Concrete" of the Special Provisions. Provide traffic control for the work described above. Traffic control shall conform to the requirements of Section '10-1.25 "Traffic Control System for Lane Closure" of the Special Provisions.
