Page 1 of 2 CITY OF DUBLIN HUMAN SERVICES COMMISSION AND HUMAN SERVICES COMMISSIONS FROM THE CITIES OF LIVERMORE AND PLEASANTON JOINT MEETING NOTICE / AGENDA WEDNESDAY, SEPTEMBER 7, 2016 TOUR OF THE NEW AXIS COMMUNITY HEALTH MEDICAL CLINIC, 5:30 – 6:30 PM JOINT MEETING, 7:00 – 8:30 PM AXIS COMMUNITY HEALTH 5925 WEST LAS POSITAS BOULEVARD, SUITE 100 PLEASANTON, CA 94566 TOUR OF THE NEW AXIS COMMUNITY HEALTH MEDICAL CLINIC, 5:30 – 6:30 PM A tour of the new Axis Community Health Medical Clinic in Pleasanton will be offered and conducted by Axis Community Health staff. The facility is located at 5925 West Las Positas Boulevard, Suite 100, Pleasanton. JOINT MEETING, 7:00 – 8:30 PM The Joint Meeting will be held at the new Axis Community Health Medical Clinic in Pleasanton located at 5925 West Las Positas Boulevard, Suite 100, Pleasanton. 1. CALL TO ORDER 2. ORAL COMMUNICATIONS 2.1 Public Comments At this time, the public is permitted to address the Human Services Commission on non-agendized items. The Commission must, however, comply with all State Laws in regard to items not appearing on the posted agenda. The Commission may respond to statements made or questions asked, or may request Staff to report back at a future meeting concerning the matter. Any member of the public may contact the Office of the Parks and Community Services Department related to the proper procedure to place an item on a future Human Services Commission agenda. The exceptions under which the Human Services Commission MAY discuss and/or take action on items not appearing on the agenda are contained in GC 54954.2(b)(1)(2)(3). 2.2 Introductions City staff will introduce their Staff members and Human Services Commissioners. 2.3 Alameda County Services in the Tri-Valley Mr. Josh Thurman, Deputy Chief of Staff for Supervisor Scott Haggerty, will present an overview of Alameda County Services provided in the Tri-Valley. 2.4 Overview of Commissions’ Work Plan and Accomplishments The Human Services Commissions will spend approximately five minutes each to discuss their accomplishments and work plans for the coming year. Page 2 of 2 2.5 Update on Dental Project AXIS Community Health and the City of Pleasanton will provide information on the status of plans for a dental health clinic in the Tri-Valley to provide affordable dental care to Tri-Valley residents. 2.6 Review of the Human Services Pocket Guide The City of Livermore will review the newly updated Human Services Pocket Guide, a guide to human services in the Tri-Valley. 2.7 Feed Tri-Valley Collaborative and Food Drive The Feed Tri-Valley Collaborative and the City of Pleasanton will provide information on the Collaborative and food drive efforts taking place in the Tri-Valley. The most recent food drive was held July 18 – 31, 2016. 3. OTHER BUSINESS 3.1 Brief Informational Only Reports from Commissioners and/or Staff, including Reports by Commission related to Meetings Attended at City Expense (AB 1234). 4. ADJOURNMENT
